Clinical Study on Malignant Hypertension

Abstract
To evaluate the clinical status of malignant hypertension in Koreans, a clinical study was done on 89 cases of malignant hypertension admitted to Seoul National University, Hospital from January 1972 to September 1975.
Following results were obtained.
1. Patients with malignant hypertension were 7 % of all the hypertensive patients admitted. Sex ratio was 2.3 to 1 with male preponderance, and age distribution showed 5th, 6th and 4th decade in decreasing frequency.
2. Of all the cases 47.1% had hypertension history of less than 2 years.
3. Systolic blood pressure was over 170mmHg (over 200mmHg in.74.2%) and diastolic blood pressure over 130mmHg {over 150mmHg in 34.5 % in all the cases.
4. Headache, dyspnea, general weakness and visual disturbance were the major clinical symptoms.
5.¢¥Papilledema was found in 53(59.6%) out of 89 cases on eye fundus.
6.. Left yentricular hypertrophy was observed in 94.4%, and normal electrocardiogram was seen in only 2.2%.
¢¥7. On chest X-ray-film cardiomegaly was found in 75.3%, ¢¥nd pulmonary congestionin 36.0%.
8. Urinalysis showed proteinuria ( 95.6%), gross hematuria- (6.7%) and microhematuria is (36.0%). Serum cholesterol was- over 200mg% in 73.8%, and the values of BUN and serum creatinine were higher than normal range in all with creatinine clearance less than 10ml per minute in 43.8%.
Anemia was found in 91.0%, and the higher the value of serum creatinine, the lower that of hemaocrit. Erythrocyte sedimentation rate was increased in 93.9%, and on peripheral blood smear eryarocyte fragmentation was observed in 9 out of O ¢¥cases.
9. Plasma renin activity was high in 4 cases and normal in 6 out of 10 cases.
10. As complications due to malignant hypertension renal insufficiency was present in all the cases
and heart failure, hypertensive encephalopathy ad cerebrovascular accidents were frequent in deceasing order. As a cause of death heart failure with uremia was 6 out of 8 cases who were dead wring admission.
